| The licensee shall ensure that the Licensing Inspection Summary is posted in a conspicuous place in the office of the home child care agency. The licensee shall ensure that the completed Licensing Checklist and the Summary of Licensing Requirements and Recommendations sheets are readily available for parents. |
| Operator to forward monthly site visit reports for 3 PHDC locations to MEDU-London office by the last day of each month beginning February 2015. The monthly site visit reports must confirm the following: 1) Poisonous substances are inaccessible to children. 2) Accurate attendance records are recorded at the PHDC location for each enrolled child. 3) Medications are administered in accordance to the label instructions and written authorization of parent/guardian. 4) Standing bodies of water are inaccessible to children in care at all times. |

Director's Approval
| Approval for extended hours and overnight care with the condition that: 1) Each approved home has a fire evacuation plan. 2) Appropriate bedding and a bed suitable for the child's age is provided. 3) The agency must ensure that each approved provider has adequate insurance in place for the provision of extended hours and overnight care. 4) The agency must ensure that spot checks of the approved homes are conducted at times when extended hours care is provided. |
