| The licensee shall ensure that the Licensing Inspection Summary is posted in a conspicuous place in the child care centre at or near an entrance commonly used by parents. The licensee shall ensure that the completed Licensing Checklist and the Summary of Licensing Requirements and Recommendations sheets are readily available for parents. |
| No child shall use the school age playground structure until a third party certified playground inspector's written confirmation all immediate health and safety issues have been addressed has been submitted to the Ministry. |
| School age licensed capacity is based on the daily use of Room 1 and the gymnasium. |

| The centre is approved to temporarily relocate their school age capacity of 30 children from Room 1 to their alternate space in the gymnasium, the preschool capacity of 24 children from Room 3 to Room 21 and the food prep area will relocate to the school kitchen for renovations for the period of June 26, 2015 to September 1, 2015, inclusive. During this time period, the centre will relocate within the school located at 25 Poynter Dr , Toronto, Ontario, on a temporary basis. The operator shall advise the program advisor via email when the program returns to the licensed premises. |
