The licensee shall ensure that the Licensing Inspection Summary is posted in a conspicuous place in the child care centre at or near an entrance commonly used by parents. The licensee shall ensure that the completed Licensing Checklist and the Summary of Licensing Requirements and Recommendations sheets are readily available for parents.
The Forum is licensed for use by a Primary/Junior School Age group of 30 children. Room 13 is licensed for use by a Primary/Junior School Age group of 23 children.
The preschool cacpity is based on the use of the preschool 1 room (24 children) and preschool 2 room (16 children).
Within the existing capacity of the child care, the operator may provide a program for 15 toddler children in the preschool (2) room when the enrolment allows. The operator is to submit written verification to the Ministry of play materials and furnishings and the transition plan prior to the change to the age group occuring.
The operator shall monitor and record the accuracy of medication administration forms weekly. Records of the weekly monitoring records shall be available for review by ministry staff at all times for the duration of the licence.
As of August 31, 2015, the operator is licensed to provide care for 1 licensed group of 26 Kindergarten children, for 1 group of 30 Primary/Junior School Age children, 1 groups of 23 Primary/Junior School Age children and must meet the staff: child ratios identified in Schedule 1 of Ontario Regulation 137/15.
Room 10 is licensed for use by a Kindergarten group of 26 children.
All rooms as set out in the shared space confirmation are approved for use by the child care program, based on licensed capacity, provided that the room or area is used by the school for children who are the same age as the age category of the licensed age group. The operator is required to keep on file at the premises an up-to-date shared space confirmation. This confirmation must include the primary use of the room during the school day and must be made available to Ministry of Education staff upon request.

Director's Approval

As of August 31, 2015, Director’s Approval is granted for the use of specified rooms/areas that may have less than 2.58 square metres of unobstructed floor space for each child in a licensed kindergarten, primary/junior school age and/or junior school age group, based on the licensed capacity, provided that the same room or area is used by the school for children who are the same age as the age category of the licensed age group.
As of August 31st, 2015 Director’s Approval is granted for mixed age grouping pursuant to ss. 8(2) and (3) of Ontario Regulation 137/15.
